Scenario to walk through:

Your name is Jamie Smith. You are a Permanency Family Case Manager at IN DCS.

When you visit a family who has recently taken placement of their 3 month old granddaughter, you find that the family is struggling to make ends meet. Grandparents are retired and on a fixed income. The additional costs of caring for an infant, is taking a toll on the family’s finances. The family needs assistance with purchasing lactose free formula. They could also use assistance with diapers, and wipes for their granddaughter who is in a size 2 diaper, and a baby gate. Grandparents are in the process of applying for WIC, but this is not currently in place. Grandparents want to be able to continue to provide care for their granddaughter, but are concerned about their ability to do so without any assistance.

The family lives in Indianapolis, at 142 Cherry Tree Rd., Indianapolis, Indiana 46205.

You know CarePortal is a great resource. After discussing CarePortal assistance with the family and getting an Authorization for Release of Information signed by the family, you are going to create a request in the CarePortal System.
