How Should You Communicate with the Family?
Communicating with the family is an important step to prepare for a successful CarePortal request. We need to ask the family for permission to use the tool and their contact information.

We need to ask the family whether it’s okay for us to advocate for their need using the CarePortal tool.
Contact Information
We need to ask the family whether we can share their name and contact number when a church responds, so the church can connect with them and drop off the needed item.
Authorization for Release of Information Form
Prior to submitting a CarePortal request, you will need to complete the approved Authorization for Release of Information Form.
